Social Wellness Check

A call to help look out for one another

The Social Wellness Check is a municipal initiative designed to provide regular telephone outreach to seniors or residents living with specific medical or physical conditions.
The service consists of a phone call from the municipal administration to confirm that the registered participant is doing well and able to maintain their independence safely.
This is a preventive and community-based service rooted in care and proximity. It does not replace medical care or public health services.
Participation is voluntary and may be discontinued at any time. The service may also be used temporarily to help residents through a difficult period, such as bereavement or recovery.

Not a Medical Service

This program does not constitute a healthcare or medical intervention service. In case of emergency or if immediate medical attention is required, please dial 911.
